Ok, from what I've seen here's a few things to try.

1. Update your drivers

2. Have you tried launching from either the SkyrimLauncer and the TESV.exe ?

3. This seems to be a popular fix --- Go to Start -> Control Panel -> Sound. In the Playback tab, right click on Speakers (or whatever has a green tick next to it), then click Properties. In the advanced tab on this new window, select 24 bit, 44100Hz (Studio Quality) in the drop down menu"

4. Try running as Admin

5. Try running under compatibility mode

For future reference (I ran into this too) the sound fix is the answer. It'll also work set to 48000, which is what I picked. The default had been sitting at 192000 for ages without a problem. I recall having run into this with Gothic 4 and having an equally tough time figuring out what was wrong. Someone on the Bethesda forum mentioned this and it worked.
